When to Seek Immediate Emergency Medical Care
  • Severe breathlessness unable to speak full sentences in one breath
  • Accessory muscle breathing (sternocleidomastoid retractions) and pulse oximetry SpO2 <90%
  • Cyanosis of lips or fingertips, or drowsiness indicating hypercapnia

In case of severe acute onset, dial 108 immediately or proceed to the nearest 24/7 Level-1 emergency trauma center.

Integrative Care Clinical Boundary & Contraindication Advisory

Inhaled Therapy Safety: Never discontinue prescribed inhaled corticosteroids abruptly in favor of untested alternative respiratory tonics. Uncontrolled severe asthma exacerbations in Delhi winter pollution can lead to acute fatal status asthmaticus. Complementary Pranayama breathing practices should strictly accompany—not replace—metered dose inhaler medications.

Clinical Background

Chronic respiratory conditions including bronchial asthma and Chronic Obstructive Pulmonary Disease (COPD) represent an enormous public health burden in Delhi NCR. Winter inversion layers, crop residue burning, and industrial vehicle emissions elevate PM2.5 levels, triggering acute exacerbations, steroid dependence, and accelerated decline in Forced Expiratory Volume (FEV1).

Pathophysiology & Disease Progression

Asthma is characterized by chronic eosinophilic or neutrophilic airway inflammation, goblet cell hyperplasia, and bronchial smooth muscle hyperreactivity. COPD involves permanent alveolar wall destruction (emphysema) and small airway narrowing (chronic bronchitis), leading to progressive, non-reversible airflow limitation.

Recognized Symptoms

  • Recurrent episodes of wheezing, chest tightness, and dyspnea, typically worsening at night or early morning
  • Persistent non-productive cough or excessive sputum production in chronic bronchitis
  • Reduced exercise tolerance and breathlessness on routine household tasks
  • Frequent respiratory tract infections requiring antibiotic or oral corticosteroid bursts
  • Acute nocturnal awakenings due to severe bronchospasm

Etiology & Risk Factors

  • Severe chronic exposure to ambient PM2.5 and PM10 particulate pollution in Delhi NCR
  • Active or passive cigarette / bidi smoking, biomass fuel smoke exposure (chulha)
  • Atopic predisposition with allergic rhinitis, eczema, or high total serum IgE
  • Occupational dust and chemical fumes exposure (silica, grain, flour)
  • Childhood history of recurrent severe lower respiratory viral infections (RSV)
Clinical Workup

Recommended Diagnostic Investigations

1
Spirometry with Pre- and Post-Bronchodilator Reversibility Testing (Gold Standard)
2
High-Resolution Computed Tomography (HRCT) of the Chest to detect emphysema or bronchiectasis
3
Fractional Exhaled Nitric Oxide (FeNO) to quantify eosinophilic airway inflammation
4
Arterial Blood Gas (ABG) analysis during acute desaturation episodes
5
Polysomnography (Sleep Study) for patients with suspected Overlap Syndrome (COPD + OSA)
Management Protocols

Evidence-Based Treatment Modalities

Inhaled Corticosteroid & Long-Acting Beta-2 Agonist (ICS-LABA)

pharmacological

Inhalation therapy (e.g., Budesonide/Formoterol or Fluticasone/Salmeterol) administered via dry powder inhaler (DPI) or metered-dose inhaler with spacer.

Long-Acting Muscarinic Antagonists (LAMA)

pharmacological

Tiotropium or Glycopyrronium added to ICS-LABA (Triple Therapy) for moderate to severe COPD or poorly controlled asthma.

Biologic Monoclonal Antibody Therapy

pharmacological

Targeted biologics (Omalizumab for anti-IgE, Mepolizumab/Benralizumab for anti-IL-5, Dupilumab for anti-IL-4/13) in severe refractory eosinophilic asthma.

Pulmonary Rehabilitation & HEPA Filtration

conservative

Structured breathing retraining, diaphragmatic exercises, and indoor HEPA air filtration during high AQI emergency days in Delhi NCR.

Patient & Clinical Guidance

Frequently Asked Questions

Clinically referenced Q&A

Q:How should severe asthma patients prepare for Delhi's winter severe AQI spikes?

Patients should optimize adherence to baseline maintenance ICS-LABA inhalers, maintain indoor HEPA air filtration with windows closed, avoid outdoor exercise when AQI >300, and carry a short-acting bronchodilator (Salbutamol) with a spacer at all times.

Q:When are biologic injections indicated for asthma?

Biologics (Omalizumab, Mepolizumab, Dupilumab) are indicated for GINA Step 5 patients who suffer frequent exacerbations or require oral steroids despite optimal high-dose ICS-LABA and proper inhaler technique, guided by blood eosinophil count and serum IgE levels.

Q:Are inhalers habit-forming or harmful for the kidneys?

No. Inhaled medicines deliver microgram doses directly to the bronchial mucosa with minimal systemic absorption, in contrast to oral steroid tablets which deliver milligram doses with systemic side effects. Inhalers are safe for lifelong maintenance.
